ATTN: FALL SPORTS---FOOTBALL, GIRLS TENNIS, CROSS COUNTRY, BOYS WATER POLO, GIRLS VOLLEYBALL, GIRLS GOLF
All fall athletes must have their full clearance by Monday, August 12th, 2013. This means, your physical must be approved by the nurse and the rest of the clearance packet approved by the office secretary, Ms. Condon. When the clearance is complete, athletes will receive a signed form to return to their coach which will allow them to participate in athletics after August 12th. No clearance=no participation=NO EXCEPTIONS!
Dates/times for athletic clearances:
Wed, July 10 1:30pm to 3:30pm
Wed, July 17 9am to 1pm
Wed, July 24 9am to 1pm
Wed, July 31 9am to 1pm
Wed, Aug 7 9am to 1pm
Thurs, Aug 8 9am to 1pm

Athletic Clearance
Thank you for selecting to print and review your athletic eligibility forms via our school’s website! We hope that you find this method easy to access and manage, as it is our goal to provide our families with the tools necessary to make the athletic process easy.
We have listed a few tips below to help you:
- Please print and READ the Athletic Handbook! There is a lot of important information in the handbook for you and your athlete. Many of the pages in the handbook will require a signature on the Parent/Student Acknowledgement/Signature Sheet stating that you have read, understand and agree with the information presented to you.
- Please make sure the packet is completed in its entirety – including phone numbers, insurance information, etc. Incomplete packets will delay the athlete’s ability to compete.
- An athlete must complete eligibility paperwork each school year. The Athletic Handbook and Eligibility Forms are updated annually at the end of April to reflect any changes for the upcoming school year.
- Please complete the Athletic Forms in a timely manner in order to expedite your athlete’s eligibility to participate in their sport season. Once a sport season has begun, any athlete that has not processed may not compete or practice after the end of the instructional day (period 6) until they have completed their athletic eligibility and have received the clearance form from the Athletic Secretary.
